The Herlyn-Werner-Wunderlich syndrome (HWW syndrome), also known as obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) is a very rare congenital anomaly of urogenital tract characterized by a triad of uterus didelphys, unilateral obstructed hemivagina, and ipsilateral renal agenesis in the majority of the cases. We presented a case of a female adolescent recently post menarche with incidental findings of renal agenesis and bicornuate uterus diagnosed on ultrasound (US). Magnetic resonance imaging (MRI) finding suggested the diagnosis of OHVIRA. Although the presentation is atypical, the anatomical finding is classical. Review of literature and the role of imaging modalities pertaining to the diagnosis OHVIRA is also highlighted.
