A RARE CASE OF PATELLA ROTATIONAL DISLOCATION AND INCARCERATION IN FEMORAL CONDYLE FRACTURE

Abstract

Patella dislocation around its longitudinal axis is rare and incarceration in femoral condyle fracture is even rarer. To date there are less than ten similar cases reported around the world. We report a 23 years old man who was involved in a road traffic accident and sustained left femur medial condyle partial articular fracture with his patella dislocated and incarcerated in the fracture site. Multiple attempts of closed reduction were unsuccessful, leading to emergency open reduction of patella. During surgery, his extensor mechanism was found to be relatively intact with only soft tissue defect over lateral aspect, of which the lateral condyle had buttonholed through it. Patella was externally rotated and incarcerated in the fracture. After disengaging the patella from fracture site, femur condyle was fixed with a locking plate and soft tissue defect was repaired. Patella tracking was restored. Six months after surgery, the fracture healed, and patient was able to fully bear weight and flex his knee up to 90 degree. Patient did not complaint of knee pain or recurrent patella dislocation.
